Medical Disclaimer and Notice

tebChart organizes health information. It is not medical advice, not a medical device, and not an emergency service. Please read this before you rely on anything in the app.

TebScribe LLC · Effective October 10, 2026

1. Information only. tebChart helps you organize and share health information. It does not give medical advice, make diagnoses, recommend or change treatment, check drug interactions or doses, or decide how urgent something is. Everything tebChart shows is for organization and communication only. Nothing in tebChart replaces a conversation with your physician, nurse, or pharmacist.

2. Emergencies. If you think you or someone else has a medical emergency, call 911 or your local emergency number now. Do not use tebChart to seek help in an emergency. Never delay, avoid, or stop care, and never skip or cancel an appointment, because of something tebChart showed, missed, or got wrong.

3. No doctor–patient relationship. TebScribe was founded by a physician. That does not make anyone at TebScribe your doctor. Using tebChart, reading our content, receiving a promo code, or messaging us on any channel, including social media, does not create a doctor–patient or any other professional relationship. We do not answer clinical questions.

4. Information can be wrong. What tebChart shows comes from:

  • what you or someone else typed;
  • paperwork, labels, and lab reports read by automated text recognition;
  • recordings transcribed by speech recognition;
  • automatic translation;
  • on-device summaries;
  • updates or charts shared by family members or caregivers.

Each of these can be incomplete, out of date, misread, mistranslated, mistranscribed, or simply entered wrong. Always check important details against the original document, the medication label, and your care team before acting on them.

5. Information from other people. Family Care Updates, shared charts, imported updates, recordings, and caregiver notes reflect what another person entered or said. TebScribe does not check, verify, or monitor them, and is not responsible for their accuracy, completeness, or timeliness, or for decisions made using them.

6. Reminders and alarms are a convenience, not a safety system. Reminders and alarms depend on the device, its settings, its battery, its operating system, and Focus modes, and they can fail or arrive late. Do not rely on tebChart as the only way to take a critical medication or keep an appointment.

7. Your responsibility. You are responsible for the accuracy of what you enter, for checking what tebChart shows, for what you share and with whom, and for having permission to keep or share another person's information. If you are a caregiver, tebChart does not change your duties to the person you care for, or their care team's duties to them.

8. Use at your own risk. To the fullest extent the law allows, you use tebChart at your own risk. TebScribe LLC and its owners, employees and contractors are not liable for any injury, illness, worsened condition, death, missed or wrong dose, missed appointment, privacy loss, or other harm arising from any of the following:

  • incorrect, incomplete, or misunderstood information in tebChart;
  • information entered, shared, or reported by you or anyone else;
  • misuse of tebChart;
  • a failed or late reminder;
  • a misread scan, transcription, or translation;
  • reliance on tebChart instead of professional care.

The full warranty disclaimer, liability cap, one-year claim limit, arbitration, and indemnity are in the Terms of Service (sections 13–16). They apply here.

9. Not a medical device. tebChart is not a medical device and has not been cleared or approved by the FDA or any other regulator.

10. Where the law gives you more rights. Some places do not allow some of these limits. Where that is the case, they apply only as far as the law allows. Nothing here limits liability that the law does not allow to be limited.
